Medical Instruction for US Embassy Visa Applicant:

Medical examinations should be conducted before the immigrant visa interview. Green Crescent Health Services and Dr. Lina Shahnaz Parveen Hoq is lawfully authorized by the US embassy, Dhaka, Bangladesh and the immigration and naturalization service of USA to conduct health screening for the students, immigrant and non immigrant Bangladesh citizen seeking US visa.

Vaccination:

According to the CDC guidelines the first dose of the vaccines is given to every applicant. These vaccines are specific for specific age groups. All the immunizations are carried out at Green Crescent Health services.

What to Bring:

  1. Original passport along with photocopy of passport (1 set)
  2. Five (5) passport size recent photos.
  3. Appointment letter along with a photocopy (1 set)
  4. Please bring along your/petitioner US address, telephone number and email address. IGRA Test Fee for child (Under 15): 7500/=.
  5. Vaccination sheet if any. Vaccination is not included in the cost of medical examination, otherwise vaccination fee is required.
  6. You can visit our Clinic Saturday to Thursday, except government holidays from 9.00 am to 5.00 pm without any appointment.

What Not to Bring :

All applicants are requested not to bring any large bags or wear unnecessary jewelry on the day of medical examination. Any accompanying person with the applicant may not be allowed in the medical center. Please do not bring along with you any other person as an attendant or escort. Only those applicants requiring special assistance are allowed one attendant to accompany them in the medical center. If you require special assistance please inform the medical center before coming for medical examination.

When Do I Get The Report? :

The result of the examination will be forwarded directly by the panel physician in a sealed envelope to the American embassy Dhaka prior to interview date.

Important Information For the Applicant:

  1. Any intentional attempt to hide medical or physical condition shall be informed to the embassy.
  2. If you are pregnant please inform the staff at the registration desk so that vaccines which are contraindicated are not given to you. These vaccines may be harmful to your baby.
  3. Complete physical examination is required by CDC and whole body shall be examined. Please take in account hygiene and cleanliness.
  4. If you or your child has high grade fever or severely ill please re-schedule your appointment and only come for medical examination when you or your child has recovered completely.
  5. Please bring your vision glasses or contact lenses at the time of medical examination. Please also bring your previous prescriptions of medications or surgical notes if any.
