Medical Instruction for Canadian Embassy Visa Applicant:

Prior the examinations following documents are required:

For citizenship and immigration of Canada the immigrants, some visitors (Students) need to undergo a medical examination before receiving a visa. Green Crescent Health Services and Dr. Lina Shahnaz Parveen Hoq is lawfully authorized by the CIC and the Canadian Embassy Dhaka, Bangladesh to conduct health screening for the students, immigrant and non immigrant for seeking Canadian visa for Bangladesh citizens.

Prior the examination following documents are required:

  1. Original passport along with photocopy of passport (1 set)
  2. Two (2) passport size recent photos.
  3. Appointment letter along with a photocopy (1 set)
  4. Medical referral letter
  5. Medical examination fee per person BDT 5800/= (if age less than 40 years), BDT 6300/= if age above 40 years, and per child under eleven years BDT 3000/=, above twelve years till 14 years BDT 4000/=.
  6. You can visit our Clinic Saturday to Thursday, except government holidays from 9.00 AM to 5.00 PM without any appointment.
